  • Med-Connect (Medicaid for Employees with Disabilities)

    Medicaid for Employees with Disabilities, also known as MED-Connect, provides medical assistance to employed individuals with disabilities. Enrollees can earn up to $75,000 per year and qualify for full Medicaid/HUSKY Health coverage.

  • Money Follows the Person

  • Community First Choice

    Are you or a loved one living in the community and need assistance to remain there? The Affordable Care Act created an optional State Plan service which will allow eligible individuals to access Personal Attendant Care (PCA) and other services and supports through self-direction.

  • School Based Child Health (SBCH)

    Medicaid School Based Child Health reimbursement information for School Districts and Providers.

  • Medicaid Nursing Home Reimbursement

    Under the Connecticut Medicaid program, payment rates for nursing homes are set on a cost-based prospective basis in accordance with Section 17b-340 of the Connecticut General Statutes and Section 17-311-52 of the Regulations of Connecticut State Agencies.

  • Elderly Simplified Application Project (ESAP)

    The Department of Social Services is pleased to announce a Supplemental Nutrition Assistance Program (SNAP) demonstration project for older adults and those with disabilities.

  • Connecticut Supplemental Nutrition Assistance Program Education (SNAP-Ed)

    The overall goal of the Connecticut SNAP-Ed program is grounded in the federal goal “to improve the likelihood that persons eligible for SNAP will make healthy food choices within a limited budget and choose physically active lifestyles consistent with the current Dietary Guidelines for Americans and U.S. Department of Agriculture (USDA) food guidance.”

  • Summer EBT - S-EBT

  • Commodity Supplemental Food Program

    The Commodity Supplemental Food Program (CSFP) works to help improve the health of low-income persons at least 60 years of age by supplementing their diets with nutritious food products provided by the U.S. Department of Agriculture (USDA).

  • The Emergency Food Assistance Program

    Nutrition Assistance programs provide food and cash equivalents to various populations DSS serves. Their goal is to promote adequate nutrition among the target populations.

  • Supplemental Nutrition Assistance Program - SNAP

    The Supplemental Nutrition Assistance Program (SNAP), formerly known as Food Stamps, helps eligible individuals and families afford the cost of food at supermarkets, grocery stores and farmers’ markets.

  • CT Medicaid Enterprise Technology System (CT METS)

    The Connecticut Medicaid Enterprise Technology System (CT METS) is a large-scale business process improvement and technology program to improve services and outcomes for residents served by Medicaid and other programs.

  • Welfare to Work (Department of Labor)

    The mission of the Department of Labor's Welfare to Work unit is to guide the provision of comprehensive, integrated employment and family support services that enable recipients of public assistance and those who are at risk of becoming recipients, to gain economic independence, and engage in life long learning.   • Acquired Brain Injury (ABI) Program

    The program is designed to assist participants to relearn, improve or retain the skills needed to live successfully in the community.
